ATHLETIC CLEARANCE REQUIREMENTS

 

Athletic Clearance is the first step to becoming a student-athlete at Corona High School. The process must be completed each school year and are valid for one calendar year based on the date of completion. Clearance Packets may be pick-up and returned to the Athletic Office during normal school hours.  The purchase of an ASB Card is recommended.  The packet includes the following.

 

• Academic Eligibility:  A minimum 2.00 GPA in all work attempted and pass at least 4 classes

• Athlete’s Registration Card and Residential Eligibility

• Preparticipation Physical Evaluation

• Medical Insurance Coverage Certification

• Authorization for Emergency Medical Care (Waiver)

• Transportation Permit

• CIF - State:  Androgenic / Anabolic Steroids Prohibition Statement

• CIF - Southern Section:  Athlete's Code of Ethics

• CNUSD Student Athletic Code

• Warning Agreement to Obey Instruction, Release, Assumption or Risk, and Hold Harmless Statement
