    It is the responsibility of the student's parents, guardian, or caretaker to obtain all of the necessary papers listed below. Registration requirements and procedures as mandated by state, county, and CNUSD are as follows:



    1.


    CALL Registrar Office for an appointment (951) 736-3315.


    2.

    PARENT, COURT-APPOINTED LEGAL GUARDIAN, OR CAREGIVER (the person with whom the student lives) along with a current photo ID (passport, driver license, employee ID) must accompany the student.


    3.

    IMMUNIZATION RECORDS: Corona-Norco Unified School District requires all of the following! No exceptions!

    • 3 Polio and 3 DPT/TD: If the last dose was given before the 4th birthday, the student needs 4 doses. The previous school has a copy or you may use the baby immunization records. There must be 4-8 weeks between the 1st and 2nd shot and 6-8 weeks between the 2nd and 3rd. Note: If the time in between doses is different than previously stated, the student needs the four doses.

    • TB Skin Test Results: If the student is entering a Riverside County School for the first time, this test must have been given within the last 15 months. When a student has a tuberculosis test, it must be "read" by the doctor/nurse two days later and have negative results; if the results are positive, the student must have an x-ray with negative results.

    • Measles, Mumps, Rubella (MMR): Student must have received one dose on or after 1st birthday. (2nd dose needed if the first dose was given before 1st birthday.)

    • Chicken Pox Vaccine (Varicella): Required for all students over the age of 12 who are entering a California School for the first time. (Students who had Chicken Pox must have physician/medical facility-documented proof of having had the disease.) If a student has already had the first dose, proof of the second dose is required from 4 weeks to 3 months after the first.

    If you are in need of any of the above immunizations, contact your family physician or the local health department at 505 Buena Vista Avenue, Corona.
     

    4.

    UNOFFICIAL TRANSCRIPT (must have all previous high school classes/grades/credits listed) and WITHDRAWAL GRADES (classes/grades that the student is taking during the present semester.) You may have them fax it to us using the fax number listed above. Eighth grade students enrolling for September classes may bring an April-June report card.


    5.

    BIRTH CERTIFICATE may substitute with adoption papers, passport, or court orders.


    6.

    GAS, WATER, OR ELECTRIC BILL with parent, guardian, or care giver's name/address on it. If the student and parent lives with another family, then the person whose name and address appears on the bill must also come to the registration appointment along with their bill and photo ID.


    7.

    HOME LANGUAGE ASSESSMENT (If a language other than English is inserted on the Home Language Survey sheet, the student must have his/her language tested at the Language Assessment Center 300 Buena Vista Avenue, Corona. You must take proof of all of the immunizations to the testing.


    8.

    IEP copy (for students in Special Ed classes) Bring the most recent copy which, if you have misplaced yours, you may get from the previous school or have them fax it to us.


    9.

    EXPELLED STUDENTS must bring a copy of the readmittance letter that was issued by the school district where the expulsion occurred. If you have not been readmitted yet, call (951) 736-5178 Ms. Chavez/ Pupil Services Department at CNUSD.


    10.

    COMPLETED CNUSD REGISTRATION PACKET



    NOTICE: In the event that it is determined a false, misleading, or deliberate omission of information is used to establish residency and secure registration in the District, such registration shall be considered null and void.
